Names and addresses of over 5,000 members people of the Society for Vascular Ultrasound database are available on a
one-time use only basis. These are delivered in a spreadsheet. The SVU list is made up of approximately 70% technologists and 30% physicians.

All orders must be made on this form and must include an explanation of how the labels will be used, as well as a copy (or draft) of the proposed mailing material.

Telephone orders will not be accepted.

Cost is $150 per 1,000 names or portion thereof. (SVU Affiliated
Chapters may receive up to two complimentary sets of electronic labels
per year of names in their regions (your state and two adjoining
states.)

All label orders must be pre-paid in full. No purchase orders are allowed.

There is a $26 charge for each sort/subset that is requested.
Please note: SVU reserves the right to refuse any list order that is for an event occurring within 60 days of an official SVU event.
